Walnut Creek, California (EastBayDaily) — A recent grant from the John Muir/Mt. Diablo Community Health Fund to the non-profit Meals on Wheels and Senior Outreach Services to reorganize and expand its senior care management program will help the agency with a long history of supporting Contra Costa seniors to adapt to the changing needs of seniors today.
According to MOWSOS Executive Director Elaine Clark, “The first year of our three-year plan of action will reorganize and expand our programs. Our newly developed client-focused assessment process will help our expanded care management team to address the specific needs of each of our clients with a more thorough and individualized approach. We expect to improve our information technology and work flow systems to serve an increased number of clients, and to make broader and more integrative agreements with local community clinics and hospitals.”
MOWSOS expects to supplement the Community Health Fund’s $161,000 grant with support from other foundations and operating funds.
“Enhancing health services for Contra Costa’s aging population is a critical priority,” said Community Health Fund Executive Director Lillian Roselin. “Meals on Wheels understands such enhancements have to last for at least a generation, which is why we are so pleased to support this project.”
